Requesting Your Counseling Records

You have the right to request access to or copies of your counseling records as permitted by applicable state and federal law.

To request records from this practice, contact our center directly. Requests may be made via email, through your portal, or in writing.

Please include your full name, date of birth, contact information, and a description of the records requested. Identity verification may be required before records are released. Records will be provided in accordance with applicable legal requirements and confidentiality laws.

Filing a Consumer Complaint
